General Patient Information

Kansas City Foot and Ankle and the Heel Pain Center of Kansas City welcomes you to our south Kansas City office. Before you set up an appointment, there are some specifics regarding patient information and information about our procedures for new and existing patients, which you should review.


New Patients

If you are a new patient, there are a few registration forms you will need to complete. As with any new appointment with a physician, It’s important that we obtain patient demographics, patient history, authorization to release medical benefits, and protected health information consent forms. You will also want to review and complete our financial policy form.

For your convenience you can complete and submit your new patient registration forms online from your computer, tablet or phone.

When you come to your first appointment, remember to bring these items:

  • Your insurance card
  • Photo ID
  • Any applicable co-pays
  • Physician referral (if needed)

When you make your initial appointment, we will send you a new patient email that includes all of the forms you will need for your first appointment and driving directions to our office.


Established Patients

If you are an existing patient and will be returning for follow-up appointments, it is important to bring your insurance card, applicable co-pay, and referral if needed.


Referrals

Insurance Referrals

Occasionally a referral is needed by your primary care physician if you participate with certain HMOs.  Some insurance plans require a referral from your primary care physician before seeing a specialist. Please make sure that you have the proper paper work, and any required co-pay when you arrive for your visit.

Referrals by Others

The nicest compliment that our practice can receive is the referral of one patient by another. Referrals by your family and friends reflect satisfaction and confidence in the care we provide. Please let us know who referred you to our practice so we may appropriately thank them.

If you were referred to our office by your physician, please let us know immediately. It is important to us that constant communication is maintained with your physician’s office and that he or she is kept informed of your progress throughout the course of your treatment.


Appointments

Because surgery and other professional commitments occasionally take the doctors away from the office, we suggest that your appointment be scheduled in advance. Our office staff is available from 8:00 a.m. to 5:00 p.m. every weekday to assist you in scheduling your office visit or surgery.

We understand the inconvenience of interrupting your day with a visit to the doctor’s office. Our office staff fully recognizes the value of your time and we will do everything possible to keep you from waiting unnecessarily. Unfortunately, our routine is occasionally interrupted by emergencies and we ask for your patience and understanding if we run behind schedule.

If you are unable to keep your scheduled appointment, we request that you contact our office as soon as possible. Your thoughtfulness will allow another patient the opportunity to benefit from our services.
