New Patient Registration Forms

Thank you for making an appointment with our office.  We look forward to meeting you and working together in resolving your foot or ankle problem.

Our goal is to make your visit as efficient as possible and still take the time where it matters most: diagnosing and treating your foot and ankle issues.  

Please complete and submit your patient registration forms online prior to your visit.  This simple step will save you time by expediting your check-in process. Our Locations

Kansas City Foot and Ankle and the Heel Pain Center of Kansas City have four convenient locations.

South Office: located near I-435 and State Line Road at the St. Joseph Medical Center. Our address is 1010 Carondelet Dr., Ste 301, Kansas City, MO 64114.  We are located in the first medical building A as you enter the medical complex off of State Line Rd.

Did you provide us with your insurance information?

So many services and products require authorization by medical insurance.  Providing us with your insurance information BEFORE your appointment allows us to see what products and services your insurance will cover before we see you at your appointment.  Again, this will allow us to concentrate our time on you and your foot issue instead of waiting on the phone with your insurance company during your appointment.

Please provide your insurance information and remember to include:

  • Name of Insurance
  • Member ID/Policy Number
  • Group Number
  • Member Service Phone Number or Customer Service Phone Number
